Public Awareness eForm

The Public Awareness eForm can be created using TransparentEarth, or out in the field using the PointMan application on a tablet or phone.

eForms that are captured in the field are immediately available for view and edit in TransparentEarth (as long as the PointMan application is 'online').

eForms that are created in the TransparentEarth application are immediately available for view and edit out in the field in PointMan.

Creating a Public Awareness eForm within Transparent Earth:

From within TransparentEarth, turn on the ProStar Locate Form Layer:

  1. Open the Layers Control panel.
  2. Click Field Data Collection.
  3. Select the check box for Prostar Locate Form.

Any existing points that have already been created will display. If no points have already been created in TE or PM, selecting this layer will enable you to see the new points you create.

  1. Zoom into the area of interest (the area in which you want to place a point).
  2. Click the Activate PointMan icon. 
      
    The New Place Mark icon displays in the upper left corner of the map.
  3. Click the New Place Mark icon (upper left corner of the map). 
      
    The New Place Mark tool icon becomes 'active', and the cursor turns into a "cross-hair" (+).
  4. Click the map at the specific point you want place your new point. 
    The New Place Mark dialog box displays. 
  5. From the System drop-down list, select Form.
  6. From the Component drop-down list, select Document.
  7. Click OK
    A list of eForms displays. 
     
  8. Click Public Awareness, then OK.
  9. A blank Public Awareness eForm displays in a dialog box within the current TE environment.
  10. Enter information in the eForm as necessary:
Field Action
Project Name Enter the Project Name.
AFE Enter the AFE Number.
Date Enter the Date of Contact.
Contractor Enter the Contractor Name.
Inspector Automatically populates with the User ID of the person logged into the system.
Location Enter the Location.
Mile Post Enter the Mile Post.
Person Contacted Enter the contact persons name.
Address Enter the address.
Phone Number Enter the phone number.
Category Enter the Category.
Handed out educational material Select check box if statement is true.
Discussed responsibility and resource available for response agency and acquainted official with our ability in responding to an emergency release and means of communication Select check box if statement is true.
 
Handed out Emergency Telephone numbers memento<o:p> Select check box if statement is true.
 
Employee Automatically populates with the UserID of the person logged into the system.
Signature Re-enter the Employee Name.
  1. Once you have completed the form, click Save
    The eForm saves and the dialog box closes. 
    In addition, a point displays on the map at the point you created the eForm. However, you must have the applicable Layer activated in order for it to display.
  2. To view the eForm you just created:
    1. Click the View Attribute Information icon, then click on the point you just created. 
      The Info box displays all information as entered into the eForm, and three options: NotifyMoveDelete
       
  3. To send a notification to a PointMan account:
    • From the Info box, click the Notify button. 
       
    • Select a name in the To field,  type a Subject, and type a Note
       
    • Click OK.
    • Notifications will display in PointMan in the users account to whom you sentthe Notification to.
  4. To move a point from one location on the map to another
    From the Info box, click the Move button. 
     
    • The map displays with the point highlighted
    • Click the Map to move the point to the new location.
    • Click Save (lower right corner).
    • The point is moved to the new location on the map.
  5. To view and edit the eForm
    From the Info box, scroll to the bottom of the Info box. 
     
    • Click the eForm link .
      The eForm opens in dialog box inside the current TE window, with the following options: DownloadEditClose.           
    • Click Edit.          
    • Complete the edits, then click Save.
  6. To download the eForm
    From the Info box, click View eFormbutton at the bottom of the dialog box. +The eForm opens in dialog box inside the current TE window, with the following options: DownloadEditClose.
    • Click Download.
    • The system opens a dialog box to browse to a specific location that you wish to save the eForm to.
      Once the file is downloaded, browse in the system file explorer to the location you saved the file and open the PDF using your preferred PDF viewer (Adobe or most internet browsers). 
      Note: Editing the eForm after downloading it is not possible, it must be edited inside either Transparent Earth or PointMan.
    • After Downloading the eForm, go back to TE and close the eForm dialog box and the info dialog box.
  7. To delete an eForm:         
    Open the applicable eForm.
    • Click the point on the map to open the Info box.
    • Click Delete.
    • Click OK at the confirmation.
      The point is deleted. The eForm should no longer show up in the search resultslist. And, the point should no longer show up in PointMan.
